Not the point above your head
The name is misleading. The Midheaven is not the zenith, the point directly overhead: it is the degree of the zodiac that was crossing the local meridian at birth, the imaginary north-south line passing above the observer. In the northern hemisphere, outside the tropics, it lies towards the south, higher or lower in the sky depending on the sign.
Another common mistake is to assume it always sits 90 degrees from the Ascendant. It does not: the angle between them varies with latitude and with the rising sign, and can stray well away from a square.
Reading it in your chart
The sign on the Midheaven describes how you move through public life and the kind of recognition you seek. A Capricorn MC is traditionally linked with ambition built patiently over time, a Pisces MC with a more inspired, sometimes less linear, sense of vocation. It does not name a job; it suggests a direction and a manner.
Then look at the planet ruling that sign and at any planets near the point itself. A planet within a few degrees of the Midheaven is the most visible in the chart, the one people tend to know you by. Astrocartography rests on the same idea: a planet's MC line marks the places on Earth where it would sit on the Midheaven, and where, traditionally, its themes become public.

Can my Midheaven tell me what career to choose?
Astrologers look at it when thinking about vocation, but it does not produce a list of jobs. It describes how you want to be recognised and the kind of contribution that feels like yours. For a fuller picture they also read the sixth house, tied to daily work, and the second, tied to money and resources. The actual choices remain yours.
Is the Midheaven the same as the tenth house?
In most house systems, including Placidus, the most widely used, the Midheaven is the cusp of the tenth house. In Equal and Whole Sign houses, however, the houses are counted from the Ascendant, so the true Midheaven can land in the ninth or the eleventh. The point itself does not change; only the house it falls in does.
Why does my Midheaven change with my birth time?
Because the local meridian sweeps through the whole zodiac in a day, just as the horizon does. On average the Midheaven moves about one degree every four minutes, so an error of half an hour can shift it by several degrees, sometimes into the next sign.
